Personalized Nutrition Support for Your Health and Lifestyle
Nutrition needs are different for every person. At ProActive Plates, I provide individualized nutrition counseling and medical nutrition therapy designed to help you better understand your nutrition needs and develop practical strategies for everyday life.
Medical Nutrition Therapy (MNT)
Medical nutrition therapy is individualized nutrition care provided by a Registered Dietitian Nutritionist (RDN) to help address specific medical or health-related needs.
Depending on your needs and medical history, nutrition counseling may support conditions such as:
Diabetes and blood sugar management
Cardiovascular health and cholesterol management
Kidney-related nutrition needs
Gastrointestinal and digestive concerns
Weight-related health goals
Other nutrition-related medical conditions
Your care plan will be personalized to your health history, preferences, goals, and, when applicable, your healthcare team's recommendations.
Cost depends on insurance coverage.

Accountability Support
Making nutrition changes is about more than knowing what to do—it’s about having the support to put those changes into practice. Accountability support provides ongoing guidance, encouragement, and problem-solving as you work toward your nutrition and health goals.
During follow-up visits, we’ll review what’s been going well, identify challenges or barriers, celebrate progress, and make adjustments when something isn’t working. We’ll focus on realistic, achievable steps that fit your lifestyle rather than perfection or an all-or-nothing approach.
Accountability support may include goal setting, progress check-ins, nutrition education, problem-solving, habit-building strategies, and personalized recommendations between visits. Whether you’re working toward improving a health condition, establishing more consistent eating habits, or simply feeling better day to day, you’ll have a supportive partner to help you stay focused and keep moving forward.

Meal Planning Support
Healthy eating is much easier when you have a plan that works for your real life. Together, we’ll create practical meal-planning strategies based on your health needs, food preferences, schedule, cooking skills, budget, and lifestyle.
Meal-planning support may include ideas for balanced meals and snacks, grocery-shopping strategies, portion guidance, meal and snack timing, recipe ideas, food preparation tips, and ways to make eating at home or on the go more manageable. We can also adapt your meals to support specific nutrition goals or medical conditions.
Rather than relying on rigid, one-size-fits-all meal plans, the goal is to give you practical tools and flexibility so you can feel confident making nutritious choices—even when life gets busy or your routine changes.

Insurance and Payment
ProActive Plates is working to establish relationships with insurance plans so that eligible patients may have access to covered nutrition services.
Insurance coverage varies based on your plan, diagnosis, benefits, and applicable medical-necessity or referral requirements. Coverage is not guaranteed, even when a provider participates with your insurance plan.
Before your appointment, it is recommended to contact your insurance company to ask about your benefits for registered dietitian services and medical nutrition therapy.
